Clinical co-management guidelines for community practices
Our clinical co-management guidelines are designed to provide community practitioners with a reference outlining best practices in dealing with common clinical challenges. Developed jointly between our faculty and community pediatricians, they provide a roadmap to efficient and effective care as well as clear guidelines outlining when to refer a patient to a CHoR subspecialist.
Note: All children are different. It is often appropriate to adapt these recommendations to the special needs of each child. These guidelines are not a substitute for thoughtful clinical judgment.
