Care Connection for Children – a statewide network of regional programs brought together by the Virginia Department of Health – helps families get the health care services, community support and resources they need for their children with special health care needs.
The team-based approach pulls together case managers, medical personnel, educators and family members to determine how children with special health care needs can reach their maximum potential.
