For parents who are concerned about attention issues in their kids, our medical team can help you learn more. ADHD – or attention deficit hyperactivity disorder – is a medical condition that affects how well your child can sit still, focus and pay attention.

Assessment process

The National Initiative for Children's Healthcare Quality Forms, also commonly referred to as the Vanderbilt forms, provide a starting point for an ADHD evaluation and diagnosis. The information provided by both you and your child’s teacher is helpful for the doctor, who also will meet with your child. If your child's doctor has instructed you to do so, please complete the appropriate forms and bring them with you
