Health care plans for students with specialized needs
We do our best to accommodate the health needs of every child. Nurses are not always available and an appointed trained designee may be responsible for administering medication or treatment.
This is why the Children's Hospital of Richmond at VCU, HCA and Bon Secours are partnering with Henrico, Richmond and Chesterfield public schools to developed standardized process to submit documents to school for the children we serve.
These care plan forms include:
- Allergy and anaphylaxis emergency plan
- Asthma action plan
- Diabetes medical management form
- G-tube physician's order
- Individualized health care plan
- Medication permission form
- Seizure action plan
Submitting your patient's care plan
Please FAX or EMAIL copies of signed care plans/orders to School Health Services by September 1st so your patient can start the 2021-2022 School Year on time!*
For CCPS, send directly to your patient's school nurse.
Schools MUST have a care plan or medical permission form from a licensed provider with parental consent to dispense medication and perform treatments.
